24/7 Complex Care

Some participants need more than a few hours of support a day. For people with high and complex needs, continuity of care, across the day, through the night, and into the weekend, can be the difference between living safely at home and being moved to residential care. First Priority Care's 24/7 complex care service is built for exactly this situation.

What our 24/7 Complex Care include

  • Personal care: showering, dressing, grooming and hygiene support
  • Clinical support: nursing procedures delivered by or under the supervision of AHPRA-registered nurses
  • Meal preparation and feeding support: including enteral feeding for participants with PEG tubes
  • Mobility and transfers: manual handling, hoist transfers and positioning
  • Communication support: for non-verbal participants or those using augmentative and alternative communication (AAC)
  • Overnight support: active night support or sleepover, depending on participant needs and NDIS funding
  • Emergency and escalation management: clear protocols and 24/7 access to clinical oversight
  • Regular care plan reviews: formal review at least every 6 months, or whenever needs change

Who is this service for?

  • Participants who require frequent clinical interventions (PEG feeding, suctioning, catheter management, medication administration)
  • Participants who cannot be safely left alone due to medical risk, falls risk or complex behavioural needs
  • Participants transitioning from hospital to home who require intensive support during the transition period
  • Participants with acquired brain injury, spinal cord injury, progressive neurological conditions or complex mental health presentations
  • Participants in Supported Independent Living (SIL) with high and complex overnight support needs

How we deliver it

Complex care requires more than having the right credentials. It requires a team that knows the participant deeply and communicates exceptionally well across shifts.

Our approach includes a comprehensive intake assessment (nursing, functional and behavioural components), a dedicated care coordinator as the primary point of contact for family and support coordinators, shift handover protocols to ensure continuity between workers, incident management and reporting aligned with NDIS Quality and Safeguards Commission requirements, and regular stakeholder communication with families, support coordinators and allied health teams.

NDIS funding

Complex care is primarily funded under Core Supports (Assistance with Daily Life) and may also draw on High Intensity Daily Activity line items, particularly where registered nurses are involved. For participants with significant needs, funding is often available across multiple support categories. We work closely with support coordinators to ensure funding is used efficiently and that care plans are aligned with NDIS plan budgets.
