Pressure injury risk assessment, prevention programs and active wound treatment, protecting participants who spend extended time sitting or lying.

Pressure injuries (also called pressure sores or pressure ulcers) are a significant risk for NDIS participants who spend extended time sitting or lying in one position. Left unaddressed, they can progress rapidly from skin redness to deep tissue damage requiring hospitalisation. First Priority Care's registered nurses provide assessment, treatment and prevention programs that protect participants at risk across Queensland.

What Our Pressure Injury Service Includes

  • Comprehensive pressure injury risk assessment: using validated tools such as the Braden Scale
  • Skin integrity assessment and ongoing monitoring for participants at risk
  • Pressure injury staging and documentation: Stages 1–4, unstageable and deep tissue injury
  • Wound care and dressing management for established pressure injuries
  • Repositioning schedule development and support worker education
  • Pressure redistribution recommendations: mattress overlays, wheelchair cushions, positioning aids
  • Skin care protocols: moisture management, barrier creams, skin hygiene
  • Nutrition assessment referral and dietitian liaison for wound healing support
  • OT referral and collaboration for seating and equipment review
  • Escalation and GP referral for advanced or infected pressure injuries

Who Is at Risk?

  • Participants who use a wheelchair or are primarily bed-bound
  • Participants with spinal cord injury, particularly those with reduced sensation
  • Participants with limited mobility, incontinence or moisture exposure
  • Participants with poor nutritional status or circulation disorders

NDIS Funding

Pressure injury nursing (both prevention programs and active wound treatment) is funded under Core Supports (High Intensity Daily Activity) or Capacity Building (Improved Daily Living).
